What is the Gall Bladder?

The Gall Bladder is a pear-shaped sac that rests beneath the right side of the liver. Its main purpose it to collect and concentrate a digestive agent (bile) produced by the liver. Bile is released from the Gall Bladder and travels through the cystic and common bile ducts (narrow tubular channels) to the small intestine. 

The Laparoscopic Gall Bladder removal surgery is usually done to treat the following conditions:

  • Gall Bladder disease
    • Gallstones
    • Infection or inflammation (cholecystitis)
    • Gall Bladder cancer
  • Biliary dyskinesia (abnormal Gall Bladder function)

What preparation is required in Laparoscopic Gall Bladder Surgery?

The following includes typical events that may occur prior to laparoscopic surgery; however, since each patient and surgeon is unique, what will actually occur may be different:

 ·    Preoperative preparation includes blood work, medical evaluation, chest x-ray and an EKG depending on your age and medical condition.

·    After the surgeon reviews with the potential risks and benefits of the operation, patient will need to provide written consent for surgery.

·    Surgeon may request that patient should completely empty his colon and cleanse his intestines prior to surgery. Patient may be requested to drink clear liquids, only, for one or several days prior to surgery.

·    It is recommended that to shower the night before or morning of the operation.

·    After midnight the night before the operation, patient should not eat or drink anything except medications that surgeon has told which are permissible to take with a sip of water the morning of surgery.

·    Drugs such as aspirin, blood thinners, anti-inflammatory medications (arthritis medications) and Vitamin E will need to be stopped temporarily for several days to a week prior to surgery.

·    Diet medication should not be used for the two weeks prior to surgery.

How well does Laparoscopic Gall Bladder Surgery Work:

 Laparoscopic Gall Bladder surgery is safe and effective. Surgery gets rid of gallstones located in the Gall Bladder. It does not remove stones in the common bile duct. Gallstones can form in the common bile duct years after the Gall Bladder is removed, although this is not common.

 Who is the candiddate for Laparoscopic Gall Bladder Surgery:

Since the advent of laparoscopic cholecystectomy, this procedure has become the gold-standard for Gall Bladder removal. Most patients with symptomatic gallstones are candidates for laparoscopic cholecystectomy, if they are able to tolerate general anesthesia and have no serious cardiopulmonary diseases or other coexisting conditions that preclude operation.

Some patients with very serious complications from Gall Bladder disease may not be eligible for laparoscopic Gall Bladder removal. In addition, patients in the third trimester of pregnancy should not usually undergo laparoscopic cholecystectomy, because of risk of damage to the uterus during the procedure.

Laparoscopic cholecystectomy in the first trimester of pregnancy remains controversial because of the unknown effects of carbon dioxide on the developing fetus.

The Laparoscopic Gall Bladder Surgery Procedure

Gall Bladder removal surgery is called for when gallstones have formed, blocking the flow of bile and leading to attacks that can cause severe pain and discomfort. With the invention of the laparoscopic procedure, the surgery can now be performed through several small openings, can be completed in one to three hours, and has a much shorter recovery time. During a laparoscopic Gall Bladder surgery, a tube carrying a video camera is inserted through an incision in the navel. The surgery is then performed under the guidance of this camera. Tiny instruments are used to cut and clip the ducts and arteries attached to the Gall Bladder and to remove the Gall Bladder from the body.

What to expect after Laparoscopic Gall Bladder Surgery?

Gall Bladder removal is a major abdominal operation and a certain amount of postoperative pain occurs. Nausea and vomiting are not uncommon. Once liquids or a diet is tolerated, patients leave the hospital the same day or day following the laparoscopic Gall Bladder surgery. Activity is dependent on how the patient feels. Walking is encouraged. Patients can remove the dressings and shower the day after the operation. Patients will probably be able to return to normal activities within a week’s time, including driving, walking up stairs, light lifting and working. In general, recovery should be progressive, once the patient is at home. The onset of fever, yellow skin or eyes, worsening abdominal pain, distention, persistent nausea or vomiting, or drainage from the incision are indications that a complication may occur. Most patients who have a laparoscopic Gall Bladder removal go home from the hospital the day after surgery. Some may even go home the same day the operation is performed. Most patients can return to work within seven days following the laparoscopic procedure depending on the nature of your job. Patients with administrative or desk jobs usually return in a few days while those involved in manual labor or heavy lifting may require a bit more time. Patients undergoing the open procedure usually resume normal activities in four to six weeks.

What are the benefits of Laparoscopic Gall Bladder Surgery?

With laparoscopic cholecystectomy, you may return to work more quickly, have less pain after surgery, have a shorter hospital stay, and have a shorter recovery time. Unlike traditional surgery, laparoscopic surgery to remove the Gall Bladder can be done without cutting the muscles of your abdomen. The incision is also much smaller. With laparoscopic cholecystectomy; you probably will only have to stay in the hospital overnight. With open cholecystectomy, you would need to stay in the hospital for about 5 days. What is Trans Vaginal Tape Surgery?

Description of the Procedure Trans Vaginal Tape Surgery:

There are several different types of suspension surgery.

Tension Free Vaginal Tape procedure:

  

Two incisions are made above the pubic bone, and one in the vagina. A nylon mesh-like tape is used to form a hammock under the urethra. This hammock closes off the urethra when a cough or sneeze pushes it open. No sutures are needed to hold the tape in place as the mesh holds onto the surrounding tissue until scar tissue grows into the mesh.

Sling procedure:

  

This procedure is performed through an incision in the vagina. Like the vaginal tape procedure, a hammock or sling is made under the urethra that prevents incontinence by closing off the urethra when pushed down by a sneeze, cough, or other stressor. The sling can be made out of a synthetic material or using the body’s fascia, which is a strong material that surrounds muscle.

Retropubic suspensions:

  

An incision is made in the lower abdomen. The surgeon places sutures (surgical threads) near the bladder neck and urethra. The threads are then secured to the pelvic bone or other structures in the pelvis. This supports the bladder by forming a cradle for it.

Transvaginal suspensions:

  

 The surgeon performs this procedure through the vagina, and through a small incision made in the lower abdomen. Sutures are placed near the bladder neck and urethra from the vagina and abdominal incision. The threads are then tied to the abdominal wall or the pelvic bone. This supports the bladder by placing it back into its normal position.

Laparoscopic bladder suspension:

  

This procedure uses 2-3 small incisions and special instruments to tie the bladder to the pelvic bone. The surgeon uses a telescope to perform the surgery. The surgery usually takes about 1 – 1½ hour.

Advantage of Trans Vaginal Tape Surgery:

 The TVT sling’s main advantage is that a sling is placed, providing new support to failed native tissue, with less morbidity than traditional sling procedures. There is no need to harvest graft material. In other words, create another incisions to take a graft from another part of the body. Therefore… less incisions and needless to say less pain. Additionally, intraoperative assessment is possible via a cough test, as the procedure is performed under local, spinal or epidural anesthesia. The patient is actually asked to cough with a full bladder at the very end of the operation and when leakage occurs the TVT sling is gently adjusted to correct the leakage. 

After Procedure of Trans Vaginal Tape Surgery:

After surgery patient will be monitored in a recovery room until patient is awake. Patient will most likely have a catheter in place to drain urine. At first, urine may look bloody, but this will resolve over time. When patient is able to empty bladder completely, the catheter will be removed. Depending upon the procedure, patient may be up and walking the same day or the day after surgery. Patient will be advised to avoid strenuous activities and lifting for three months, so that sufficient healing can take place.
